Definition
A process insurers use to evaluate whether a requested medical treatment is necessary and appropriate, sometimes resulting in a denial or modification of the treatment plan.
Why It Matters
A UR denial doesn't necessarily end the matter — most states have an appeal process specifically for disputed treatment denials.
